How do I do that?

By continuing to meditate, even after you've completed the chakra puzzle.

You'll need to do it in a specific place, however.

Where?

Outside Meadow's cabin.

The achievement may only unlock if she's actively meditating (as seen through the window) when you put on the pendant.
