What do I need to do?

Complete the musical menhir puzzle after learning the rough location of Malgrim's tower.

How do I do that?

After adding the stone ring to the 'main' menhir and hearing the tune, you need to copy it on the menhirs further down the path.

Play the menhirs in this order: 3-4-3-3-1-2-5. The achievement will unlock after a cutscene at the top of the hill.
