This missable achievement is unlocked when you enter a radioactive area without adequate protection.

Which area?

All of the computers in the game will warn you about this area - it's spewing dangerous levels of radiation.
Sector 6.

How do I avoid protection?

A hazmat suit that can protect you from radiation poisoning is in a side-room near Louna. If you don't want the hazmat suit, then avoid the box with the crutches in that room.

How do I get radiation poisoning?

Walk near the train in sector 6, and Aino will comment that she doesn't feel well. The achievement will unlock as she draws a radioactive symbol in her book.

What does radiation poisoning do?

It doesn't impact the ending or make the game harder, but will force the bad ending and make you go back and get the hazmat suit to proceed if you've killed both Esko and Viljo.
