Switch back to Rosa and look at the door to discover that it has a floor-level slot like a doggy door that's presumably used for sliding in a food tray once in a while.

Scootch Rosa over to the door and click on the slot to have her hold it open with her foot, then have Joey blow the scalpel into the room. Rosa can now pick it up and use it to cut herself out of her bindings. (Considering how straitjackets are designed, I'm not quite sure how; maybe she has prehensile toes or something.) (But never mind that.)

But she still can't get out of her locked cell. Maybe there's a vent or something behind the padding on the wall.

Use the scalpel on the walls to discover a loose pipe and a vent. Use the pipe on the vent to open it and exit to --

-- the storeroom. (And I'm also not going to ask why there's a ground level vent from a cell to a storeroom.) Here, open the white locker to find Rosa's clothes and glasses. Yay! You can now go to reception, where you can use the key from the locker to operate the elevator and try to stop Madeline.
